Know someone who may benefit from additional support? Individuals, families, healthcare providers, and community partners can refer someone to EPIC. Our team will review the request, connect with the individual, and help determine appropriate next steps.
Instead of asking people to fit into programs, EPIC builds services around each individual's unique journey. Some individuals receive one service. Others benefit from multiple services working together. Our care teams coordinate communication, reduce duplication, and ensure every member experiences seamless, person-centered support.
While each service serves a different purpose, they work together to provide coordinated, person-centered support that improves health, independence, and quality of life.
Enhanced Care Management (ECM) provides intensive care coordination for individuals with complex medical, behavioral health, and social needs. Care Advocates work alongside members to develop individualized care plans, coordinate providers, remove barriers to care, and improve overall well-being.
Housing Supports address social factors that influence health and well-being. These services help individuals obtain the resources necessary to remain healthy and independent within their communities.
(Coming Late 2026)Community Health Workers build trusted relationships within the community, helping individuals navigate healthcare systems, understand available resources, and overcome barriers to accessing care. Members cannot be enrolled to both CHW and ECM/CS at the same time.
EPIC currently offers two direct Care Services pathways: Enhanced Care Management and Housing Supports. In late fall, EPIC will expand Care Services with Community Health, providing community-based support focused on health education, navigation, connection to resources, and helping individuals address health and social needs.
