EPIC Connect is EPIC's digital engagement system that connects individuals, families, community partners, and members with EPIC services, programs, resources, forms, and ongoing participation.
EPIC Connect is expected to launch in September. Until then, you can connect with EPIC by phone or email, or submit our current 365 EPIC Interest Form online.
Once available, EPIC Connect will make it easier to request or refer for services, complete enrollment forms and intakes, securely upload and access documents, register for programs and events, receive EPIC updates and opportunities, and stay connected with our team—all in one place.
Use the EPIC Connect Form to connect yourself or someone else with EPIC. Tell us what services, programs, or resources you're interested in, and our team will review the information and follow up to help determine the next steps including making you an EPIC Connect Account.
The EPIC Connect Portal, made possible with support from CCAH, provides a convenient way to stay connected with EPIC. Through the Portal, participants can access forms, programs, resources, events, documents, and ongoing engagement opportunities—all in one place. Complete the EPIC Connect Form to get started and connect with our team through the Portal.

EPIC Central is our facility in Monterey off of Garden Road. We welcome walk-ins for those interested in our services.

Integrated Care is central to our continuum of services. Your provider can help you start this process. Give them our information if you think collaborative care is best.

EPIC currently offers two direct Care Services pathways: Enhanced Care Management and Housing Supports. In late fall, EPIC will expand Care Services with Community Health, providing community-based support focused on health education, navigation, connection to resources, and helping individuals address health and social needs.
Below, you can learn how to request Care Services for yourself or submit a referral for someone who may benefit from support.
Getting Started is Simple! Complete the EPIC Interest & Referral Form or contact EPIC directly. Our team will learn about your needs, review eligibility, and help identify available Care Services. If a service is appropriate, we'll guide you through enrollment and work with you to develop a personalized plan.
Connecting someone with EPIC Care Services is simple. Whether you're helping someone you know or making a referral as a community provider, you can complete the EPIC Interest & Referral Form, call or email our office, or submit a referral by fax.
EPIC is committed to creating opportunities that support individuals, families, and communities. EPIC Families is currently launching as our first Community Initiative, with EPIC Thrive and the EPIC Market & Resource Center planned to follow in the coming months.
EPIC Connect is expected to launch in September 2026 and will provide a new, centralized way to connect with EPIC Community Initiatives. Until then, individuals and families interested in participating in EPIC Families can complete the current EPIC Families Interest Form to get started.
Coming Soon! Joining an EPIC Community Initiative will be easy through the new EPIC Connect Portal. Once launched, you can use the Portal to explore available initiatives, express interest, complete forms, and stay connected with EPIC. You can also contact the EPIC office by phone or email, or visit us during regular business hours.
Coming Soon! Referring someone to an EPIC Community Initiative will be simple through EPIC Connect. Complete the electronic referral form, and our team will take it from there. If you're a community partner or provider, you can also let us know if you'd like to collaborate and complete any necessary documentation to support ongoing coordination.
