While each service serves a different purpose, they work together to provide coordinated, person-centered support that improves health, independence, and quality of life.
Enhanced Care Management (ECM) provides intensive care coordination for individuals with complex medical, behavioral health, and social needs. Care Advocates work alongside members to develop individualized care plans, coordinate providers, remove barriers to care, and improve overall well-being.
Housing Supports address social factors that influence health and well-being. These services help individuals obtain the resources necessary to remain healthy and independent within their communities.
Community Health Workers build trusted relationships within the community, helping individuals navigate healthcare systems, understand available resources, and overcome barriers to accessing care. Members cannot be enrolled to both CHW and ECM/CS at the same time.
Instead of asking people to fit into programs, EPIC builds services around each individual's unique journey. Some individuals receive one service. Others benefit from multiple services working together. Our care teams coordinate communication, reduce duplication, and ensure every member experiences seamless, person-centered support.
EPIC currently offers 3 different direct care services including Enhanced Case Management, Housing Supports, and Community Health. Select the previous links for more information on each service. Below is information on how to both request services for yourself and also to request services for someone else.
Getting Started is Simple Submit a referral or contact EPIC. We learn about your needs. We review eligibility. We recommend available services. If appropriate, care begins with a personalized plan.
Getting support for someone you know or if you are a local provider is simple. You can call our office, email a request, or fax a request.
Enhanced Care Management (ECM) is available for eligible Medi-Cal members who are experiencing complex medical, behavioral, or social challenges. Eligibility is determined by the member's health needs and specific populations identified by the California Department of Health Care Services.
If you're unsure whether you qualify, our team can help determine eligibility and assist with the referral process.
Individuals or families who are experiencing homelessness or are at risk of losing housing while managing complex health or social needs.
People with repeated emergency department visits or hospitalizations who may benefit from coordinated care.
Individuals receiving specialty mental health or substance use treatment who also face complex health or social challenges.
Adults who need additional support to safely remain living in the community.
Residents who are ready to safely transition from a skilled nursing facility back into the community.
Children enrolled in CCS or CCS Whole Child Model with additional health or social needs.
Children and youth currently or previously involved with foster care or child welfare services.
Pregnant and postpartum individuals eligible under California's Birth Equity population of focus.
Adults and youth returning to the community after incarceration who have ongoing medical or behavioral health needs.
